- Please fill out the intake packet PDF from this page.
- On the portal, start a new message to Dr. Collins and attach your completed PDF.
- You will also receive a link via email or text to complete additional new patient paperwork tasks for Emmaus Health Partners. Please complete both sets of tasks.
Existing patients - please also complete this intake packet one time and send it to Dr. Collins in the portal if you haven't already done so.
